Who Can Enroll?

Our Case Management Program connects you with a licensed nurse to help navigate and coordinate your care. We offer support to plan participants who meet one or more of the following criteria:

• Frequent inpatient admissions
• Readmission within 30 days
• Frequent ER/Urgent Care visits
• Transplants (organ, stem cell)
• Discharge planning
• High-risk maternity
• NICU admission

Talk to Licensed Nurses—Not Call Centers

The hardest part of managing your care isn’t always the diagnosis—it’s knowing what to do next.

If you decide to enroll in the program, your Nurse Case Manager will work with you to build a personalized care plan based on your goals and health plan benefits. This includes:

• Regularly discussing your health care goals and progress with you and your doctors.

• Pointing out treatment options and helping you get answers to questions.

• Assisting in making appointments, explaining what to expect and helping prepare for your visits.

• Suggesting when to get a second opinion.

• Providing educational materials and directing you to community resources.

• Coordinating access to in-network clinicians, including dietitians, pharmacists and social workers.

• Recommending plan benefit options for achieving wellness goals.

Once enrolled in your health plan, you will receive access to your plan documents, which contain detailed information about plan benefits, exclusions and limitations.
